As a parent you know the TEAM is more than just the girls on the field..select which areas you would be willing to help out the team.  Just because you select an option doesn't mean you will be expected to fill the role.  Just want to know which areas you would feel comfortable helping if needed. *
What other sports does your athlete play and please list any ROUGH dates when those sports take priority over softball practices and games.  EX Your daughter plays basketball and December-February she will miss conflicting softball practices or workouts for pre-scheduled basketball games. *

As a parent / guardian of the above child, I give permission for emergency medical treatment of my child for illness or accident if I cannot first be contacted. I agree to release from liability and to hold harmless the Oregon Silver Bullets Fastpitch Softball, its coaches, board members and agents, as well as Westview High School, Tualatin Hills Park & Recreation District (THPRD) and the Beaverton School District (BSD), from any and all claims and liabilities arising out of or in any way connected with this activity. *
