Statement of Assurance: I attest that all statements on this request form are accurate.  I acknowledge that I have read  Regulation 7510 Use of School Facilities  in accordance with the provisions of N.J.S.A. 18A:41-7.c and will abide by said rules and regulations set forth.  I am aware of how to evacuate the building should such a need arise. In signing this application I understand that I will be held responsible for communication with the Custodial Department, and that our organization is responsible for any damages incurred.

I understand requests must be submitted 20 working days prior to each date requested and that the school district reserves the right to cancel, amend or change any approved application. Any fees for this will be submitted within five (5) days of the approved application.  I will wait for an email for approval from Berkeley Township School District for my request.  *
