The Music and Dance in the Park PM session is now full. If you would like to be put on the waitlist for the afternoon session, please email chanelleb@lampchc.org with the following information:

1. Adult first and last name (only registered adults may attend the program)
2. Child first and last name (if you have more than one child, please register both of them)
3. Child date of birth
4. Phone number
5. Are you currently registered in any other programs that LAMP EarlyON offers? Please note that priority will be given to families not currently registered in any other programs.
6. Please share any information that will improve your child's experience in the program (e.g. interests or dislikes)
7. Any additional comments
8. Before participating in the program, you will need to pick up an activity bag which has all of the activities we will be doing during the program. Please check the pickup location that is most convenient for you.
