                                  (g)   Individual Refuge taking. Prior registration required; a Dharma name will be given to those taking refuge (Please submit TWO photos (30mm x 20mm) to our office (28/F Pacific Plaza, 410 Des Voeux Road West, Hong Kong) by 17 May 2019 for our preparation of Refuge Certificate. Please also provide your FULL NAME in English and Chinese in the registration form below. You are required to wear white garment during the ceremony.) 皈依儀式。欲皈依者須預先於網上報名，並於二零一九年五月十七日前提交近照兩張 (30毫米 X 20毫米) 至本中心 (香港德輔道西410號太平洋廣場28樓)，以便中心製作皈依證，亦煩請於下列表格提供中英文全名。皈依者須於當日穿著白色衣服，並將獲發法名。)
