Student Travel Out of State Form
The Belle Vernon Area School District needs to comply with the new order enacted by Dr. Rachel Levine, the PA Secretary of Health, on November 17, 2020.  Effective November 20, 2020 parents/guardians of students traveling outside of Pennsylvania must complete this form notifying the Belle Vernon Area School District PRIOR TO leaving Pennsylvania.  After completing this form, your child's principal and school nurse will be notified.  For a student to return to be physically present in school or participate in sports, he/she will need to provide documentation to his/her principal of a negative COVID-19 test within 72 hours PRIOR To entering Pennsylvania or he/she will need to quarantine for 10 days upon return to Pennsylvania and participate in remote instruction only.  Only students over the age of 11 are required to follow this order.
