This form will not directly accept payments, but there are two options for finalizing your order:

    Type in the total amount due for the video in the field titled: "Would you like to make a donation?"
    (We will take care of converting the amount paid into a video payment.)
    Do not forget to submit this order form after you've completed your online payment.

  2. Mail/drop off payments in-person by cash or check to:
