#TeamVaccineMetuchen will begin taking requests from ALL ADULT RESIDENTS (18 and over) on Wednesday, April 14.

- Appointments will be made on a first come, first served basis
- While we will book your appointment as close as possible, some sites may be up to an hour away by car
- You may be contacted by one of our volunteers by phone, text, or email
- The information we are requesting is safe to provide
- We will never ask for your social security number or financial information
- We regret that we are unable to offer transportation at this time
- If you do not keep your booking you are responsible for canceling and/or rescheduling

Your submission must meet both of these guidelines or it will be rejected: (1) You must currently reside in Metuchen, and (2) You must be 18 years of age or older *

Submissions requesting appointment dates more than two weeks in advance will be canceled and will need to be resubmitted.
