Attendees are required to complete this event evaluation form by MAY 30, 2021 to receive a statement of credit. Your attendance will be verified; if you did not register for the event, you will not receive a statement of credit.

Statements of credit (CEU certificates) will be emailed to the address provided below by JUNE 13, 2021.
