Parents and Guardians,

Please complete the following form to RSVP your attendance at the High School Academic and Athletic Banquet on April 29 so that meals can be ordered.  

This year the Academic and Athletic Banquets will be combined into one banquet.  A meal has traditionally been provided by the school at the Academic Banquet, but not the Athletic Banquet.  This year a meal will still be provided by the school district at the combined banquet.
