WWJH Choir Student/Parent Contract and Handbook Agreement

Choir Contract/Handbook Agreement due date: Friday, August 30th.

Participating in choir should be a rewarding and enjoyable experience. In order for this plan to have its greatest effect, we need your support. Please discuss the choir expectations as described in the WWJH Choir Handbook with your child taking time to be sure your child understands all of the choir eligibility guidelines and how they affect participation in choir activities. This form must be completed with electronic signature on or before the due date for a student to remain enrolled in the WWJH Choir program.

Ensure you have completed the following:

1. Read through the entire choir handbook with your child. Be sure you understand all expectations, procedures, and eligibility, then complete and submit the form below.

2. Register with Cut Time and make sure contact information is up to date. See Cut Time instructions here:

3. Fill out and return the required medical release form.

4. Review the choir calendar and make note of required events.

5. Complete the parent volunteer background check form at www.tomballisd.net

Cut Time Acknowledgement

For parents of students 13 years of age or under only.

We use an online database system - CutTime.net - to communicate with parents and students. Students under 13 need parent permission to access their account. Please indicate permission below. *

By checking this box, I grant permission for my child to access his or her Cut Time account online and receive email communications from the WWJH Choir.
