There are tasks detailed below that need completing before we can book you to start.

The tasks will take about an hour and must be done with the person with diabetes if they are over the age of 8 years

We cannot order the pump or book the start until the tasks have been completed.

If you cannot compete a tasks contact us and we will arrange a time to work through them with you. 

If you get more than one question wrong for the video questions, you will need to re-take the questions. The questions are easy if you watch the full video. The score will be revealed on completing the form.

Create a CareLink Personal account to be able to upload your pump. 

You will need a computer or laptop to be able to upload your pump. If you cannot access a computer please let us know below.

b.   Create an account

c.  Select "Patient"

c.  Follow through the set up

d.  Create a username and password and complete further details

e. Complete verification by getting code via the email

f. Login with username and password

g. Click on upload now and install the Medtronic uploader

Injections example:

Tom is 14 years old. He takes 34 units of Lantus. Therefore his background is 34units

Pump examples

Go into the pump setting to find total daily insulin and get the average of the last 14 days and see how much is basal insulin.

Tom usually has 10 units of NovoRapid insulin with breakfast, 7 units with lunch, 3 units with a snack and 12 units with his evening meal, this totals 22 units of bolus insulin.

This is not essential, but will help you feel very confident when you start the system.

You will be using the Guardian 4 sensor with no calibrations so ignore the calibrations part
