Additional Yearbook Order Form (21-22)

North Glendale PTO is purchasing one yearbook per family. If your family would like additional yearbooks beyond that one, please fill out this form with how many more books and which child should receive the additional book(s).

Once you have completed this form, you have two options for payment:
• Paypal — Log in to your account and search @northglendalepto or go to https://www.paypal.com/paypalme/northglendalepto and select “Standard Transfer” (not “Instant Transfer”) so a fee is not applied.
• Check — Send a check payable to "North Glendale PTO" to the office for the additional books.

Remember your family's first book IS PAID FOR by PTO. We will deliver yearbooks to the oldest child in the family. If you have more than one child at NG and would like a yearbook for each child, or additional books for your family, please order here and pay PTO $12 per yearbook.
