The J. Moss Foundation has been awarded with the CDC Health Equity Scholarship that allows our organization to train prospective Lifestyle Coaches AND seasoned Lifestyle Coaches that serve priority populations at no cost (Lifestyle Coach Training or Advanced Training).
Although our Lifestyle Coach Training scholarships have been fully allocated, we still have a limited number of spots available for our on-demand Advanced Training courses!
Priority populations include Medicare beneficiaries; men; Black or African American, Asian American, Hispanic, American Indian, Alaska Native, and Pacific Islander persons; and people with visual impairments or physical disabilities.

Requirements:
Applications can be considered from CDC-recognized organizations with at least pending recognition. Scholarship support is allowed for an organization that has newly earned pending recognition, but has not yet trained their lifestyle coaches, provided their programmatic focus includes serving priority population(s).
In addition, per the scholarship application, “It is expected that trainees funded under this award mechanism will lead a cohort within 6 months of training completion.” Scholarship funds cannot be used by an organization that has not applied for CDC recognition.
If you are applying as a prospective Lifestyle Coach, you must work for a DPP recognized organization or your organization must have submitted the "Diabetes Prevention Recognition Program Application" within the last 15 days.

You must have a Coach ID specific to the organization providing DPP. (A Coach ID will be assigned by the organization to uniquely identify and track
Lifestyle Coaches. The Coach ID must be included on all session attendance records generated for
individual participants. The Coach ID may not be based on a coach’s name, social security
number, or other PII)
