Lions After School club application form - September 2023
Please complete this form if you wish to apply for a place at our after school club.  Latest pick up is 5.50pm.  

Please complete one form per child.

Daily fee for regular sessions is £15.00 per child.  Ad hoc sessions are charged at £16.00.

All fees must be paid in advance of sessions taken.  Fees are still payable should your child not attend a session.  Please ensure that you advise the school office or Lions directly on 07542 793393 if your child is not going to attend a session.  

Collection Information: 
  • Late collection will attract a fine of £1.00 for every minute past 5.50pm

  • If there is a change of collection arrangement to someone not listed on Arbor as authorised to collect, you will be required to notify the school office in writing via lions@chasebridge.richmond.sch.uk 

  • Let us know the first and last name of the person whom you are giving permission to collect your child 

  • Verbal confirmations will not be accepted 

  • You should also indicate if this is an on-going arrangement or a one off situation 

  • If the arrangement is likely to be frequent, this information should be added to Arbor
