I hereby acknowledge that the activities involved with team/company and class participation at The Hive Dance Co. are inherently dangerous and involve the risk of serious injury.
I hereby indemnify and save and hold harmless The Hive Dance Co. its instructors, employees, or agents from any loss, liability, damage, or cost they may incur arising out of or related to The Hive Dance Co. events, whether caused by the negligence of The Hive Dance Co. or otherwise. I assume the risk of harm or injury which may occur to the participant as a result of participating in the above-named activity. I hereby release The Hive Dance Co. from any liability, costs, and damages resulting from this individual's participation. I agree the above-named minor(s) have my consent to participate in The Hive Dance Co. events and activities. I also give my consent for The Hive Dance Co. to seek emergency treatment for the minor(s) if necessary, and I agree to accept financial responsibility for the costs related to this emergency treatment.
