McKinley Elementary Yearbook Committee will use photos of McKinley Elementary students who are involved in various school related activities as well as a class photo. This is your opportunity to submit your formal release allowing us to include them in the yearbook.

By signing below you are granting permission for McKinley Elementary Yearbook Committee to use photographs of your child/children in the McKinley yearbook. This is your opportunity to opt-out for any reason.

We want to create an inclusive yearbook that will provide families with a memorable token of their elementary school years. There is no obligation to buy a yearbook, however without your permission, your child cannot be part of the yearbook.

Please submit one response per child. If you have more than one child at McKinley Elementary school, you will submit a separate response for each child.
