We appreciate you helping us to be thoughtful and professional in preparing your experience by reading and acknowledging the following:

I plan to participate in mindful meditation, yoga therapy and/or tai chi services (hereinafter collectively referred to as “Activities”) offered by The Regents of the University of California, on behalf of UC Irvine, Susan Samueli Center Integrative Health Institute, including its employees and independent contractors.

I acknowledge that the Activities may require mental and/or physical exertion, which could potentially result in stress or injury. It is my responsibility to inform the instructor(s) if I have any condition or limitation that may put me at risk.

I agree to follow the directions given to me by the instructor(s). However, it is my responsibility to monitor my own abilities and make appropriate decisions for my own wellbeing, which may include adjustments or ceasing the Activities.

I realize that although people may experience stress reduction while participating in the Activities, there may also be times when they feel more stressed. This can happen as awareness grows and they begin to sense more clearly the stress they are facing.

I understand that I am advised to consult with a healthcare professional (e.g. a physician or a therapist) to determine if it is the best time to enroll and/or whether I need support while participating in the Activities, especially if one or more of the following are present: moderate to severe health condition, injury, pain, substance abuse (including early stages of recovery), untreated psychosis or mood disorder, anxiety, panic disorder, PTSD, acute depression, self-harming behaviors (e.g. cutting), suicidal thoughts, recent loss, or anything else that might be relevant.
