Overnight Field Trips - COVID-19 Test Results
All San Domenico students and chaperones participating in an overnight field trip are required to submit this form by 5:00 pm on the day prior to trip departure.

Any student or chaperone with no response will not be allowed to attend an overnight field trip.

Day Students: Form must be completed by a parent/guardian.
Boarding Students: You may complete this form yourself.

  • Nausea or vomiting
  • Diarrhea 
Any student or chaperone with COVID-19 related symptoms may not attend an overnight field trip and must inform the appropriate SD administrator immediately:
