DEADLINE IS SEPTEMBER 3, 2024

For the ad to be included in the program, you must do 3 things:
1. Fill out and submit this form.
2. Submit payment via Cheddar Up
3. Submit text in the form below, and email images/ads to fmhstheatrepublicity@gmail.com

Business 1/4 Page Vertical -- 2.25"w x 3.75"h ($67) -- also includes full color ad on website

Business 1/2 Page Horizontal -- 4.75"w x 3.75"h ($129) -- also includes full color ad on website

Business Full Page -- 4.75"w x 7.75"h ($235) -- also includes full color ad on website with clickable link to your website (if applicable)

Ad Content *

We are happy to create the ad for you with your photo(s) and text. If you choose to create the ad yourself, please submit actual size, camera-ready artwork, 300 dpi, PDF or JPEG. Please submit photos or ad art via email to fmhstheatrepublicity@gmail.com by September 3, 2024.

Create the ad for me. I am submitting text and photo(s) via email. I understand I will not see a proof. (Please keep ad size in mind when selecting text and images. For example, a quarter page is small; so keep text short.)
