Proceeds from sales will go to either individual accounts (name to be entered at the end of this form, or into the general youth account if no name is entered).

Order deadline is Sunday, October, 30th; wreaths will be available for pick up on Wednesday, November 23rd by 4:00 pm by the church garage.
