Please fill out this form in its entirety. If you are filling out the form for a patient over 18 years of age, the superbill can ONLY be sent to the patient unless there is a release on file. Although we try to get your requests done as quickly as possible, please allow a two week turn around on superbill requests. Once completed, you will receive a message that your superbill has been uploaded to your patient portal. Thank you!
