This form will be taken as an official entry into the Champion of Champions Competition 2024. Please submit an official entry form including all players you are entering.

Entries must be received by May 17th.

An invoice will be sent to the provided email address with a confirmation of your entry.  

Please complete all details of all players in the intended playing order.
