I hereby authorise the staff of Allegro Child Care Centre Ltd to administer Tylenol to my child with a temperature that exceeds 102 degrees F. I also agree that with accordance of this agreement I will come to pick my child up within 30 minutes and follow up with a doctor’s visit. The staff of Allegro Child Care Centre Ltd. agrees to call that parent or emergency contact person before any Tylenol is given. The staff will also take the temperature again every ten minutes so the parent will have a more accurate temperature when they arrive.
