Step 1: Watch one of the linked videos.
Step 2: Complete Section 2 "Personal Information" which includes your PPID number.
Step 3: Select the video you chose from the drop down menu in Section 3.
Step 4: Answer the PDE required questions in Section 4.
Once you submit this form for the video you chose, you may select another video to watch and repeat the steps for however many of the videos you choose. Each video is approximately 15 minutes in length. You will receive 30 minutes of Act 48 for each video you watch and complete the form. Note: You must complete this form in its entirety for each video you choose in order to receive credit for that video. The opportunity will be available until April 30, 2024. Your hours should show in your PDE PERMS account by mid May.
