This the is registration form to attend the virtual GEWWELS girl's ministry for girls ages 12-25.

GEWWELS is a bible-based ministry to help lead girls through different phases of life as they transition from adolescence to teen to young adult.

Through virtual gathering, mentoring, and structured workshop, young ladies learn about topics that include anger, abuse, abandonment, rejection, self-esteem, image, identity, boys and dating, peer pressure, health & hygiene, self-care, leadership, resume writing, effective public speaking, job preparation, interview skills, money management, time/task management, cultivating a relationship with Jesus.

One of our goals is to assist young ladies with identifying her "star" quality (what makes her stand out) by way of gifts and talents, then help her cultivate that gift to share with the world. We aim to help young women shine in purpose, shine on purpose and shine for God's purpose.

Girl Talk Sessions:

Girls are separated into groups based on age with a mentor: 12-14, 15-16, 17-19, 20-21, 22-25. She stays in this group until her age shifts her out of the group. The mentor will take notes on what the girls talk about to keep in her records.


RULES:
Girls are expected to adhere to and abide by the following rules with an understanding that a breach if following these rules will result in dismissal from the program.
1) Confidentiality: what is discussed in GEWWELS sessions must remain in the GEWWELS sessions. We do not discuss other girl's issues with anyone. If this is broken, it requires immediate removal from all GEWWELS meetings, and that young lady is no longer allowed to attend anything associated with the GEWWELS ministry.

Client’s Right, GEWWELS are assured of confidentiality with limitations.
There are, however, important exceptions to confidentiality that are legally mandated. These exceptions include: (1) that Team Taylor Ministries & Mentors notify relevant others if we believe that a client has an intention to cause harm to another individual or them self; (2) that Team Taylor Ministries & Mentors report any incidence of suspected abuse, neglect or molestation of minors or the elderly; (3) that Team Taylor Ministries & Mentors release our records to the court if subpoenaed; (4) that Team Taylor Ministries & Mentors have consent from both GEWWEL and parents before any information is released at the request of either or both parents and/or their respective attorney.

Mentoring & Coaching Information and Disclaimer GEWWELS Mentoring Disclaimer
Team Taylor Ministries and GEWWELS mentoring requires that this Disclaimer be read and signed by all parties receiving coaching/mentoring prior to the first coaching session. 1. Team Taylor Ministries and GEWWELS Mentors are not licensed therapists or counselors; the ministry is led by licensed pastors, ministers, and qualified mentors. 2. Team Taylor Ministries and GEWWELS Mentoring is a female-to-female mentoring relationship, with an emphasis on information about God’s Plan for life. 3. All discussions that take place during the Virtual GEWWELS sessions will be held in the strictest of confidence. 4. Any staff member, who serves as your child's mentor / coach(es), or Team Taylor Ministries cannot be held responsible in any way for any decision a person makes as a result of the coaching or mentoring she has received. 5. GEWWELS mentors are employed by Team Taylor Ministries (Ronald & Chara Taylor) cannot be held liable in any way for any of the results of any coaching received or any decisions made, or any actions taken by any client as a result of these decisions.

Payments and Fees:
GEWWELS Virtual Girls Ministry is $360 per year (which is $40 per month) to attend. Registration cost of $25 is required for paperwork. Fees take care of mentors and materials. The girls will also have an opportunity to give a donation each month to the ministry. The donations received from the girls will go towards helping a family who has a need, or as incentive/reward for completing tasks/assignments or winning small group challenges. This giving opportunity is a chance to teach the girls about giving, being appreciative of services they receive, and being responsible / good steward with money.

Your electronic signatures also indicate that you are making a voluntary, informed choice to enter a coaching relationship with Team Taylor Ministries & GEWWELS Mentoring and its appointed staff and you have read and agree with the contents of this form.

REGISTRATION FORM INFORMATION: This form requires parental information for all girls regardless of age. If your child is under the age of 18, we need consent to talk about certain topics. The reason we need parental information for all girls regardless of age is that in the unlikely event there is an emergency, we need contact information. We will use parental information for emergencies or if we believe a girl is in danger.
