Please use this on-line form to register for in-person worship no later than Wednesday of the week prior to the worship service you wish to attend. We have limited seating capacity due to physical distances requirements available on a first come, first served basis.

1. Are you comfortable being seated three feet from people outside your household *
2. When you arrive for worship, can you show ONE of the following: A) QR code showing proof of being fully vaccinated at least 14 days ago. B) QR code showing proof of a medical exemption. C) Authorized proof that you've had COVID-19 in the last 90 days and that you've since been cleared by the local public health unit. *
3. Are you currently experiencing one or more of the symptoms below that are new or worsening? Symptoms should not be chronic or related to other known causes or conditions: Fever and/or chills, cough or barking cough (croup), shortness of breath, decrease or loss of smell or taste, fatigue, lethargy, malaise, muscle aches/joint pains. For children under 18: also - nausea, vomiting and/or diarrhea. *
4. In the last 14 days, have you travelled outside of Canada AND been advised to quarantine (as per the federal quarantine requirements)? *
5. Has a doctor, health care provider, or public health unit told you that you should currently be isolating at home? This can be because of an outbreak or contact tracing. *
6. In the last 10 days, have you been identified as a "close contact" of someone who currently has COVID-19? (If public health has advised you that you do not need to self-isolate, select "No".) *
7. In the last 10 days, have you tested positive on a rapid antigen test or a home-based self-testing kit? (If you have since tested negative on a lab-based PCR test, select "No".) *
Your registration will be confirmed by phone. If you are unable to attend, please notify us in advance if possible. A signed WAIVER OF LIABILITY (available at the church or www.ByronUnited.ca) is required the first time you attend. One waiver is required for each adult. If minor children are attending, both parents must list the children's names on their waiver forms. You only need to complete waivers on your first attendance at a worship service.
