BY SUBMISSSION OF THIS FORM I ACKNOWLEDGE THE FOLLOWING: (If your child is a virtual client, we understand that certain things may not apply.)• Parents can apply for enrollment of their child in Valor & Grace Literacy Cove Summer Series by completing the Enrollment Application or Online Registration Form and paying for the entire summer series or sessions of your choice . •Per day charge is $20.00 without a contract or on daily basis. •Initial Enrollment is contingent upon receipt of completed enrollment application, including the signed fee agreement, registration fee and facility use waiver. •Payment is accepted via cash, card, check or Venmo (Venmo tag: @Aundrea- Smiley) there is as $25.00 fee for all returned checks (in addition to the amount of the check)• If your child is picked up after their scheduled pick up time there will be a late charge assessed of $1.00 for every one minute. •If the child is taking antibiotics for an illness or he or she has a fever (over 100F), the child will not be permitted to return to the facility for 24 hours after conditions has returned to normal. We may require a doctor’s decision as to whether or not the child is contagious. •I hereby give permission to have pictures and/or videos taken of my child in the program setting for publicity purposes. • I hereby give my consent for all the staff members of Valor & Grace Literacy Cove to call medical practitioner or ambulance for my child in the case of accident or illness and/or administer first aid/CPR, if I cannot immediately reached. •I release Valor & Grace Literacy Cove and all its employees/volunteers from ALL Liability for accidental injury to my child while in the care of Valor & Grace Literacy Cove . *
