Chicago Public Schools (CPS) Statement of Assurance for Homeschooling
The Illinois School Code (105 ILCS 5/26-1), case law, and CPS policy 601.6 permit a parent or legal guardian to educate his or her child at home if instruction is given in the English language. The parent(s)/legal guardian(s) is responsible for providing a child with an organized educational program and using appropriate materials and teaching methods in order to satisfy statutory requirements.

If parents or legal guardians choose to educate their child at home, CPS requests that they complete this form. This documentation is requested to be filed with CPS annually for each child.

This form is collecting the parent/guardian's email address in order to email a copy of the responses back to the parent/guardian as documentation.

​If applicable, please provide your District school with a copy of this documentation. Please also keep a copy for your records. 

​Instructions for CPS Schools: Please print and file a copy of this form in the student's Cumulative Folder and input Leave (Exit) Code 40 - Transfer to Home Schooled in Aspen.

Once you have completed this form, you may begin homeschooling immediately. There is no approval process.
