I (parent name listed below) will be providing transportation for my child (student name listed below).

I, as the parent or legal guardian, have notified the coach personally and understand the school will be released from liability as soon as we leave the facility.

BISD School Board policy FMG Regulation requires written parent permission for your child to ride to and from the event with you, an adult that you designate, or to be able to drive themselves if legally licensed.

Student safety is our highest priority and we cannot release a child to another adult who is not on the emergency pickup list in Skyward or has not been designated on this form.

This form has to be completed 48 prior to the event. Your electronic signature is verification of your understanding and agreement to comply with our BISD school Board policy, FMG Local.
