We are very sorry to see you go and hope you consider coming back to 423 Yoga in the future. We have a few questions that we need you to answer before you say goodbye. Your cancellation will be processed at the end of your next billing cycle, as long as you provide it at the start of the billing cycle. If you send this form less than thirty (30) days from your next billing cycle, you will be charged for an additional billing cycle and your Agreement will remain active until the end of that billing cycle.
