Dublin HIIT Training Group PAR-Q and Client-Trainer Agreement

Being more active is very safe for most people. Please read them carefully and check yes or no for the question if it applies to you. For most people physical activity should not pose any problem or hazard. PAR-Q is designed to identify the small number of adults for whom physical activity might be inappropriate or those who should have medical advice concerning the type of activity most suitable for them. Common sense is the best guide in answering these few questions. Please read them carefully and check the correct answer opposite the question if it applies to you.

If you answer YES to any of the below PAR-Q questions, then you are required to gain consent from your doctor before participating in physical activity. Tell your doctor about the PAR-Q and the questions to which you answered YES.

If you answer YES to any of the below PAR-Q questions and wish to participate in physical activity without your doctor’s consent, you do so at your own risk.

If you answer NO to all of the below PAR-Q questions, you can be reasonably sure that you can participate in physical activity.
