This form will ask for information about your event details including date/time/location and requests for support from the CFA staff including marketing, box office, front of house and technical needs.

Please keep in mind the CFA requires at least six weeks' advance notice to support events fully as our staff time and spaces (and oftentimes audiences) are shared between dance, theater, music, art and art history faculty and students.
