April 13th, 2:00 - 6:00 PM. Youth will meet at Gateway Fellowship at SAVA to begin. Parents will pick up their children at 6:00 PM at Mountain Mike's, 2069 Arena Blvd., Sacramento. You MUST RSVP by Thursday, April 11th, so we can be sure to have enough drivers!

I give permission for my child(ren) to participate in the Goose Chase in the Natomas area, and I agree that the
Legacy Leadership Team will not be held responsible for any injuries or illnesses that my child sustains during the activity.

I hereby authorize adult Legacy leaders, as agents of myself, to provide routine health care, and to seek emergency medical treatment, if deemed necessary. In the event that I cannot be contacted during an emergency, I authorize the physician or hospital selected by the leaders of Legacy to provide treatment, including hospitalization, for my child(ren).
