Please complete this form for any NYC Yellow School Bus or MetroCard request for the 2022-2023 school year (NYC Residents Only).  Please complete by May 1, 2022.  (Nassau County Residents must contact their school district directly by April 1.  Additional information can be found on the OLS school website.)
Eligibility for NYC Residents
Grades K - 2 - must reside 1/2 mile or more from school - eligible for NYC Yellow School Bus
Grades 3 - 6 - must reside 1 mile or more from school - eligible for NYC Yellow School Bus
Grades 7 & 8 - must reside 1/2 mile or more from school - only eligible for MetroCard
