Everything contained within this application is confidential, and can only be seen by Disability Services staff.
If you are a new student, this completed form and documentation verifying
your disability from a medical professional such as a counselor or
doctor must be submitted before you can receive accommodations.
Our accommodation process is tailored to each student's individual circumstances, but students must inform Disability Services of their disability and provide proof of diagnosis to receive accommodations. Accommodations are not issued retroactively, and accommodations received in the K-12 system or at another college are not guaranteed. We do not provide assessments or evaluations and cannot diagnose disabilities.
