We are so glad you found us and want to follow your artistic impulses with The Creative Center's Art Workshop Program! Please fill out this Participant Welcome Form to the best of your ability, so that we can learn a bit more about you and your creative interests. Even if you have been a participant with us for years, please take the time to submit this vital information to help us serve you best.

The Creative Center at University Settlement is a 501(c)(3) nonprofit organization dedicated to bringing the creative arts to people who have lived with cancer or chronic illness, caregivers, and healthcare workers -- through all stages of life.

Eligibility to become a Creative Center participant: if you are currently living with or have lived with cancer or chronic illness, if you are or have ever been a caregiver to someone living with or who has lived with cancer or chronic illness, or if you are or have ever been a healthcare worker.

In order to provide successful, meaningful, and direct services using our well-practiced methods, we structure our programming with our participants in mind. Please let us know how you fit into The Creative Center community so that we are able to share the most relatable programs and resources with you!
