Informed Consent and Acknowledgement - I hereby give my approval for my/my child’s participation in Sporty Girls Netball training. I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless Sporty Girls Netball and all its respective volunteers, agents, and representatives, from any and all liability for injuries to said player arising out of travel to, participating in, or returning from this event. There is a risk of being injured that is inherent in all sports activities, including netball. Some of these injuries include, but are not limited to, the risk of fractures, paralysis, or even death. In case of injury to said player, I hereby waive all claims against Sporty Girls Netball including all coaches and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct this event. Furthermore, all images and videos of Sporty Girls Netball events are the property of Sporty Girls Netball and can be used for any promotional consideration. I hereby state that I have carefully read the above waiver. Acceptance and understanding of this agreement are hereby acknowledged. I have read and agree to the Informed Consent and Acknowledgement.
