Please submit this form if you are requesting to use our Facilities for an event at The Lakes Church (for events needing promotion, please fill out the Event & Communications Request Form).

Dates, room reservations, tech needs and special setup requests will have to be approved before an event is officially put on the calendar. Facilities Request forms must be submitted at least six (4) weeks prior to an event in order to give adequate time for the process. Once forms have been evaluated by church staff, you will receive an email confirmation within one week that your event has been booked, or if additional information or change is needed, you will be contacted by a staff member.
