I am 18 years of age or older and have read, understood and agreed to the terms below.

I agree to participate in a trial adoption of the pet described above and understand there is a trial period of 2 weeks beginning from when the pet arrives at my home.

If my adoption application is successful, I agree to inspect the pet on the first meeting. Furthermore, I agree that by proceeding with the trial adoption I am indicating my acceptance of the pet in its current condition, health and behaviour. I have not relied upon any representation by Animal Welfare Carers - Kalgoorlie (AWC) in deciding to adopt the pet.

If the pet is injured or sick during or after the 'Settling In/Cooling Off Period', he/she will receive appropriate veterinary care and I agree to pay the costs of any such veterinary care.

If during the trial period, I feel the pet is unsuitable for any reason, I will contact AWC and arrange a mutually convenient time to return him/her and all relevant paperwork to AWC.

The pet will, at all times, wear a collar with an identification tag attached during the trial period.

The pet will not be allowed off my property or allowed in any unsecured area without a collar and lead during the trial period.

I acknowledge that I have answered all questions above truthfully and completely.

I understand that the animal will remain the property of AWC until the adoption fee is paid in full and the AWC 'Transfer of Ownership' form has been signed and returned to AWC.

I acknowledge that it is a condition of adoption that all AWC pets are desexed at the earliest suitable age.

If applying to adopt a young puppy from within a reasonable travelling distance of Kalgoorlie, I understand that the adoption fee is $600 unless otherwise stated and that if my application is successful, I will be provided with a desexing voucher and agree to take the puppy for desexing at Kalgoorlie Animal Hospital between the ages of six to eight months old (6-8mo).

If applying to adopt a young puppy from an area beyond a reasonable travelling distance of Kalgoorlie, I understand that the adoption fee is $600 unless otherwise stated and that if my application is successful, I will be required to take the puppy for desexing at my chosen vet hospital between the ages of six to eight months old (6-8mo). I understand that once the spay/neuter procedure has been carried out and I have provided proof of desexing to AWC in the form of a sterilisation certificate and/or provider receipt, I will be eligible for AWC’s ‘Out of Town’ $100 desexing refund.

If applying to adopt an adult dog, I understand that the adoption fee is $450 unless otherwise stated and that if successful, the dog is required to have desexing, microchip implant and vaccination carried out prior to being adopted.

If applying to adopt a cat or kitten, I understand that the adoption fee is $290 unless otherwise stated and that this fee covers the cost of all vet work in accordance with the ‘Cat Act 2011’ requirements for change of ownership, which are desexing, microchip implant and initial F3 vaccination.

I acknowledge that AWC strongly encourages that cats be kept either indoors or in a secure cat run at all times. I understand that if my application is successful and I choose differently, AWC recommends that my new cat should be vaccinated with the F4/F5 vaccine prior to having outdoor access.

I understand that if I am unable to travel to Kalgoorlie for collection, the animal will be delivered to me at a mutually agreeable meeting point by an AWC transport volunteer. I understand that such arrangements are made on a case by case basis and although every effort will be made to meet everyone’s needs, plans may be subject to change at little to no notice. I understand and acknowledge that transport arrangements are not within AWC’s control and that free transport with a caring volunteer is both a favour and a privilege, not a right.

I indemnify and hold AWC free and harmless from all liability arising out of any and all claims, demands, losses, damages, action, judgements of every kind and description which may occur to or be suffered by me, members of my household or any third parties by reason of activities arising out of this agreement.

AWC reserves the right to refuse any applicant and will not enter into any further correspondence on its decision.
