I ACKNOWLEDGE THAT BY SIGNING THIS DOCUMENT, IAM AGREEING TO RELEASE FALCON SCHOOL DISTRICT #49,(THE DISTRICT), AND ITS EMPLOYEES, DIRECTORS, OFFICERS,SERVANTS, VOLUNTEERS, AND AGENTS FROM LIABILITY.  IHAVE THEREFORE BEEN ADVISED TO READ THIS DOCUMENT CAREFULLY BEFORE SIGNING IT.

By signing this Release, I expressly assume these risks on behalf of my student whether such risks are known or unknown to me at this time. Further release the District and its employees, directors, officers, servants, volunteers, and agents from any claim that I may have or my student may have against them as a result of physical injury or illness incurred during the participation by my student in this evenVprogram. This release of liability shall include (without limitation), any claims for negligence or breach of warranty that Imay have against the District or its employees, directors, officers, servants, volunteers, or agents. This release of liability is also intended to cover all claims that members of my family or my estate, heirs, representatives, or assigns may have against the District or its employees, directors, officers, servants, volunteers, or agents. Except to the extent that a claim might be based upon the sole and exclusive negligence of the
District, further agree to indemnify and hold the District harmless from liability resulting from the participation by my student in this event/program.
