Student Absence Form
We realize some absences are unavoidable due to sickness, emergencies and other circumstances.   We also know that when students miss to much school, regardless of the reason, it can cause them to fall behind academically.   The NYCDOE defines a student who misses 10% of the school year (or a total of 18 days by the end of June) as a chronic absentee.      

Special note:  An absence is an absence, even if it is considered excused.

Please complete this form whenever your child is absent.
