Please use this form to let us know if your swimmer is looking to unenroll from Storm or take a break.  We ask that you give us 15 days notice before the end of the month to ensure we can suspend or cancel your membership in a timely fashion.  Below are the fees associated with requests that come in after this timeframe:

15 or more days notice: No charge
5-14 days notice: $25.00 Charge
4 or less days notice: $50.00 Charge

This form is for those signed up for our seasonal membership.  Our Fall/Winter Session runs from September through February and our Spring Session runs from April through July.
