Reminder: Team Fees must be paid in FULL by due dates above (or $50 late fee applies).  Invoices payable online will be sent after this registration form is submitted.

Tuesday and Thursday 7-9 PM at Ed Fountain Park
Saturdays 8-10 AM alternating between Teton Trails and Discovery Park

This Registration Form must be completed for each player in order to play for  REBEL Lacrosse (even if you have played with Rebel before).

SUMMMER FINANCIAL AGREEMENT :: I agree to pay the full amount for REBEL Summer.  I I understand that my team fee is due IN FULL by deadlines above, and if I fail to pay my daughter will not be allowed to practice or play at any tournaments until I have met my financial obligation. I further understand that failure to pay by dues dates will result in a $50 late fee. Refusal to pay in full prior to our first tournament will result in forfeiture of tournament roster placement. Lastly, I understand if I choose to send my daughter to any of the tournaments as a SOLO, I will have an additional SOLO fee that I must pay prior to the event. My electronic signature below certifies that I have read, understand and agree to the conditions and terms of this agreement as stated. (Payee's full name must be typed below - First and Last) *
PARENT SIGNATURE :: I have read through the Parent & Player Packet and my (our) electronic signatures below indicated that I (we) fully understand and agree to abide by all expectations that have been laid out in the packet. Furthermore, I (we) understand that any Mean Girl behavior is ground for immediate termination of myself and daughter from REBEL Lacrosse. I (we) also understand that I (we) must pay ALL team and uniform fees by the listed deadlines in order to avoid any late fees. Lastly, I (we) understand that my behavior is a direct representation of REBEL Lacrosse. I (we) understand that it is okay to have a bad day every now and then, but if my (our) behavior is consistently negative, The REBEL Way / REBEL Lacrosse reserves the right to excuse me (us) and our daughter from the program without refund, and/or not invite my (our) daughter back to play for any upcoming seasons. OPTIONAL - SOLO TRAVEL OPTION :: We offer this option to help provide a way for the girls to attend the tournaments even if Mom or Dad cannot attend. The fees for this option is $250/solo tournament. The fee covers car rental, gas, hotel, and chaperone fees ONLY. You will still be responsible for purchasing flights and sending your daughter with food/spending money. If flights are necessary, SOLOs are REQUIRED to travel on the same flight as the REBEL Coach/Chaperone.  Please let us know if you are going to be sending your daughter as a SOLO to any of the events this summer.  A solo contract will shared to communicate expectations of solo travelers.  IMPORTANT :: Once you have selected to send your daughter as a SOLO you are responsible for SOLO fees, even if you choose to attend later on.
