This form is designed for those who are interested in serving in Louisiana with SendMeMissions for Disaster Relief. These trips are designed for able bodies ages 16+ (Minors 15 and younger will be considered on a case by case situation and must be accompanied by at least 1 parent)

If your spouse or family will be going as well, please fill out this interest form per person (including children/minors)

All expenses are paid by SMM (lodging, food & water are included). *If you have food allergies, please plan on bringing your own meals & snacks.

Due to COIVD-19 all volunteers should not plan on serving if they have shown any symptoms of illness or been around anyone with symptoms of COVID-19. All other safety precautions should be adhered to while serving.

More details will be available as the trip gets closer and will be provided via emails.

Once this trip is confirmed a Liability Form will also be required from each person.
