THIS FORM NEEDS TO BE COMPLETED FOR EACH TRANSCRIPT REQUEST:

I understand that consent is voluntary and that it may be revoked in writing at anytime.  I will only need to provide the name, phone number and purpose of releases in writing to the office where my original transcript request was submitted.  All scores from outside testing agencies such as ACT, AP, PSAT and TSI must be requested from the testing agency.  

University or other entities that request official copies of the AAR directly from the school district are responsible for obtaining authority from the student for release of such records.
