Baptism Registration
Please note, registration is NOT complete until you've submitted:
• your child's birth certificate
• certificates from godparents (married individuals will be asked to provide us with their Catholic Marriage Certificate and single persons will be asked to provide us with their Baptismal Certificate)
• paid $10 baptismal stipend per child.  Cash, Checks and Major Credit Cards are accepted.
  Checks can be made out to: St. Philip the Apostle Parish
• our Baptism Coordinator has contacted you to confirm that your form has been successfully processed.

Be sure to read the Baptism and Godparent Requirements if you haven't yet done so.

Contact our parish office at 920-468-7848 if you have more than one child to register for baptism or he/she/they are older than 7 years old.

                    or during the 10:30 am Mass on Sundays with Fr. Juan Zarate
English ─ Sundays at 9:30 am with approval first

IMPORTANT: Choose a date at least 2 months from time of registration to have a couple of opportunities to attend the Baptismal Prep Session (Plática Pre-Bautismal).  Baptismal Prep Sessions are usually held in Spanish on the 3rd Thursday of every month in church at 6:00 pm for parents and godparents who've registered in advance.  No walk-ins allowed. Baptismal prep (English nor Spanish) sessions are NOT offered in December.
