Rescue Pet Vet Kitten Adoption Form
Thank you for considering adoption.  Our goal is to set our kittens up for a successful adoption into a forever home.  Filling out an application is the first step in the adoption process. You are not required to adopt a kitten once your application is submitted and this application does not guarantee any particular kitten.  

Applications are reviewed in the order received.  

If you are interested in a particular kitten and we are unable to accommodate your request we will keep your application.

Keep in mind the requested information below helps us, not only screen potential adopters, but to help "match" our kittens for a successful adoption.  

Once your application is submitted.  RPV has been very fortunate that our kittens receive a high volume of interest. We do our best to contact everyone but if you do not hear from us within 5-7 days that is indicitive of a large volume of applications and/or an adoption pending.

All kittens are spayed or neutered, are FeLV/FIV negative, have a current (age appropriate) FVCRP vaccination, are microchipped, and have a current dose of Revolution.  Adult cats or older kittens will have a current Rabies vaccination.

***In person "meet and greets" are required***  No exceptions.
