PVS Athlete Travel Assistance - Application
This simplified form will be used to apply for PVS Athlete Travel Assistance.  Coaches submitting this request for assistance must verify that the athlete meets the requirements for assistance (https://www.pvswim.org/coaches/reimbursement.html).  Per PVS Policy, the request must be submitted within 30 days after the completion of the meet for which travel assistance is being requested.  Eligibility will be verified.

If the request is based on an appeal of the PVS meet participation requirements, the reason for the appeal must be clearly stated.

If you are requesting travel assistance for more than six athletes, you may submit multiple forms.  If you are submitting for athletes from multiple sites that need to be paid separately, please indicate that on the form.
