The purpose of this survey is to help to assess qualitatively your pastor's effectiveness in pastoral ministry at the Knoxville Grace and Lenoir City Seventh-day Adventist® Churches from January to December 2021. Your responses to the following statement will aid in the identification of areas of strength and/or improvement in his professional development and personal growth as a pastoral leader. The survey should take you 15–20 minutes to complete.

While this survey is anonymous in order that you may respond openly and honestly, your answers will be shared with your pastor. If you wish to keep your responses anonymous, please do not include any identifying information. Also, please ensure that your responses are formulated constructively and manifest Christian maturity, even when suggesting areas of needed growth for your pastor. Essentially, build up instead of tearing down your pastor (Prov 12:18; Eph 4:29; 1 Thess 5:11; Jas 1:26).

Each statement in the primary areas of assessment is accompanied by a rating scale of 1 to 5. Please respond to the following statements to the best of your ability, indicating to what extent you agree or disagree with each of the statements. Near the end of the survey, you will be given the opportunity to share your own written feedback with your pastor.
