COVID-19 / MN YOUTH SPORTS WAIVER. The enrolled participant do for themselves, heirs, executives, and administrators waive and release any and all rights and claims for damages we may have against the Anoka-Hennepin ISD#11, or other district sites for any and all injuries suffered by me or my child while participating in and/or arising from the use of any facilities of the above mentioned school district. By the signature below, we also recognize the contagious nature of COVID-19 and voluntarily assume any risk to which we may be exposed to from said enrollment. Waiver of Liability, Assumption of Risk, and Indemnity Agreement: Activity: Blaine Boys Basketball Skills Clinics, Summer Youth Camp, Summer Breakdown Tournaments and Open Gyms. This form must be completed by the participant and his/her parents and approved by Blaine Boys Basketball Program before a participant is allowed to participate in this activity. I hereby give my consent for the above named participant to participate in the above names activity. Assumption of Risks: Participation in Activity carries with it certain inherent risks that cannot be eliminated regardless of the care taken to avoid injuries. The specific risks vary from one activity to another, but the risks range from 1) minor injuries such as scratches, bruises, and sprains to 2) major injuries such as eye injury or loss of sight, joint of back injuries, heart attacks, and concussions to 3) catastrophic injuries including paralysis and death. *
